Registration for the Twist 2013 Elite Hockey Camps is NOW OPEN. Our Camps completely SOLD OUT in 2012. To avoid disappointment, book your camps as early as possible.

Registration Forms
To register, please download our registration form above and submit via e-mail (will need to print and scan in), or fax or mail your forms to Twist Sport Conditioning. All registration forms need to be completely filled out before submitting and MUST include a valie method of payment.
Method of Payment: Visa, MasterCard, or cheque (payable to Twist Sport Conditioning). All programs must be paid in full upon registration.

Cancellation Policy
No refunds within 30 days to the start of the camp. In the event of an accident or injury, athletes must provide a medical statement from their doctor and will be provided a full credit minus a 10% administration fee. All cancellations outside of 30 days to the start of the camp date will be provided a refund minus a 10% administration fee.
